Even thought we live as a married couple, his health insurance won't cover me until we are married.
Long story, but I have bad teeth. Partly a birth defect and an idiotic old dentist who put braces on me with out checking my old med records out. Anyway, I want my issues fixed before the wedding.
Should we just courthouse wedding and not tell anyone, and just have the wedding next year as planned?

It can be expensive, though because domestic partnerships are not recognized by the federal government. So not only is the partner's monthly portion after tax, taxes have to be paid on the employer's portion (that goes towards partner's insurance), just as if it were income.
It might not be too bad for just dental, depending on what the employer's portion is.

Well looking at the paper work, they pay 80% of the minor stuff (extractions, cleanings) and 50% of major work including prosthetics.
so as opposed to over 5k, it would only be like 1.5k. So it seems like a good plan.

Have you checked around to see what kind of a discount you could get on your own as a self pay? You would be surprised how stuff like this can be negotiated just by asking the question.

First, I would look into an individual plan. A lot of times they run fairly inexpensive.
Second, if no luck with the dental plan I would look into a vocational dentistry or dental hygienist program around you. Often times you can get work done by students at little to no cost, especially if what you have is rare.
Third, Do the courthouse wedding but look into why you had reservations that made you post here looking for advice and if u could live with them.

I can tell you my exact reservations about it. My identical twin ran away joined the Army and married a man her family to this day has not met. Not even me. It would break my moms heart if she found out we had a courthouse wedding before our ceremony. But even she mentioned a Ch wedding for insurance, is not so bad since I am having the ceremony later.

Mama fox, have you ruled out "domestic partner"! My company insured DH for a whole year that way before we got married. Then when we were married I just sent them the license to update his file.
I would also suggest making
I would also suggest making sure that what you are going to have done falls into the 80% category vs the 50% category. Dental insurance is notorious for classifying as much as they can into 50%. 50% is still better than 0% but it does make a difference if you are budgeting especially with a wedding coming up.
